Application
Once you have passed the written test, you will be given an application to present to the DMV.
Birth Certificate
Must be an original, no copies will be accepted.

Social Security Card
Present your social security card to get permit.
Proof of Address
Documentation with YOUR name and address - not your parent's name(s). **Class schedule works great!
Legal Guardian
Your guaridan will have to accompany you to the DMV in order to sign for permission to get your permint.
